Reviewing US Military’s Targeted Strikes

The US military has a dark, well-hidden history of conducting drone strikes on houses or vehicles in Afghanistan, Syria, and Iraq, that supposedly held ISIS bombs. However, a number of reports have revealed that these claims were proven to be false, costing thousands of innocent lives that happened to be near the strike zone. Why has the US military hidden these reports and what needs to be done to prevent innocent lives from being lost in the future?
